This is a Case Management programme to assist clients with who have mild to moderate health conditions who would like to return to employment, education or training. Participation is voluntary and entry to the programme is based on the findings of an initial assessment process managed by Lanarkshire Health Board (Salus) and consent of a GP.
The assessment involves an individual health and action plan that reflects mental and physical needs physical needs using a range of assessment tools including CORE (Clinical Outcomes for Routine Evaluation) and COPM (Canadian Occupation Performance Measure).
As part of case management services, Health for Employability will deliver appropriate interventions to provide participants with the capablility to self manage their condition. Interventions include:
- BACP approved Counselling Services
- Physiotherapy
- Alternative Therapies, e.g. Bowen Therapy
- Post intervention Case Manager Support
Health for Employability works closely with the key employabilty agencies in South Lanarkshire and targets workless participants in receipt of JSA or Income Support who present mild health issues that may be considered a key barrier to progression.
